Physical Evaluation & Medical History Forms


Per TAPPS and HCA guidelines, students participating in athletic activities must review both the Physical Evaluation and Medical History Form. Parents and students must provide honest and thorough answers and return the completed forms to the school office. Student safety is of the highest importance, and these steps will help regulate and protect our students competing in sporting and extracurricular activities.

Transfer Student Additional Forms


These forms must be completed and submitted to TAPPS for approval prior to participation in TAPPS Sanctioned activities, for students transferring to a TAPPS High School.

All TAPPS student-athletes must complete the TMS Eligibility Process before participating in athletics and other eligible activities.
